The VNA continues to visit patients in their homes with the added benefit of enhanced monitoring via home Telehealth between, home visits. This added ability to see patients between visits allows the VNA to reinforce the education process and to maintain closer contact with patients.

We can provide the physician with reports on the patient's condition, including graphics of their vital signs at intervals of the physicians choosing. Receiving these reports can assist the physician in making changes to the patient's treatment plan.

What Is Interactive Home Telehealth?

Interactive home Telehealth allows the VNA the ability to visit and monitor patients in the comfort of their homes. Telehealth offers a system that features live video/audio and medical peripherals such as:

bullet Blood pressure unit
bullet Digital scale
bullet Pulse oximeter
bullet Glucose meter

How Will Patients Pay For Home Telehealth Technology?

Any patient who is referred to our for home care services and meets the service criteria will be home Telehealth candidates at no additional cost to the patient.

How Does Telehealth Work?

A Patient Station is placed in the patient's home while the Central Station is located in the VNA office. The nurse uses the Central Station to collect information from the monitoring station or conduct a video visit by calling the patient. They can see and hear each other while the clinician obtains visual and verbal information, gathers patient vital signs and documents clinical findings with the unit camera.

The system functions over ordinary telephone lines.

 
Joan Shaw, CHN

Nursing Case Manager and Telehealth Nurse Coordinator, with the VNA since 1986. She serves as a case manager, coordinator of the Home Telehealth Program and is available for Telehealth educational programs.

Julia Elacqua, CHN

Nursing Case Manager, with the VNA since 1995. She is a case management specialist and also serves as a Telehealth nurse.
